Background

This randomized clinical trial investigated the efficacy and safety of Boswellia serrata + Apium graveolens against knee osteoarthritis and cartilage degeneration.

Method

In this study, 62 participants orally received a novel nutraceutical blend featuring Boswellia serrata Roxb. gum resin (300 mg) and Apium graveolens L. seed extract (250 mg). The enrolled subjects underwent a series of assessments, including visual analog scale (VAS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), Physician’s Global Assessment, the six-minute walk test, knee X-rays, the Knee injury and Osteoarthritis Outcome Score (KOOS) questionnaire, and the Functional Assessment of Chronic Illness Therapy - Fatigue scale (FACIT-F) score. At baseline and after 90 days, biomarkers linked to inflammation, and cartilage deterioration and regeneration were also estimated in serum and urine.

Result

Participants taking the nutraceutical experienced sustained symptom relief, with notable reductions in pain, stiffness, and swelling. Key biomarkers of inflammation (serum interluekins [IL-7, IL-1, IL-6], high-sensitivity c-reactive protein, tumor necrosis factor-alpha, erythrocyte sedimentation rate) and cartilage degradation (serum c-terminal telopeptide of type II collagen [CTX-II], cartilage oligomeric matrix protein [COMP], matrix metalloproteinase-3 [MMP-3], urinary C-terminal telopeptide of type II collagen urinary [CTX-II]) decreased markedly in the treatment group when compared to baseline and placebo.

Furthermore, markers of collagen synthesis (like serum N-propeptide of collagen IIA and procollagen-type-C propeptide) were elevated, indicating cartilage regeneration. Throughout the 90-day period, no adverse effects were witnessed in clinical exams, biochemical, hematological, or electrocardiogram analyses.

Conclusion

The combination of Boswellia serrata and celery extract presents a promising and safe herbal alternative for tackling osteoarthritis, offering not only symptomatic relief but also promoting cartilage health and regeneration.
