An omeprazole–domperidone combination provides substantial symptomatic relief in APD while supporting improved glycemic control in patients with T2DM in real-world clinical practice.

Acid peptic disease (APD) continues to impose a considerable gastrointestinal burden on patients with Type 2 diabetes mellitus (T2DM), with symptoms such as abdominal pain, epigastric burning, nausea, altered bowel movements, flatulence, and loss of appetite frequently affecting daily functioning and quality of life. Although Proton pump inhibitors (PPIs) combined with prokinetic agents are routinely used to manage acid-related disorders, evidence supporting their effectiveness in diabetic populations has remained limited. Therefore, investigators evaluated the effectiveness and safety of an omeprazole–domperidone combination in patients with T2DM and APD receiving stable oral antidiabetic therapy.

The retrospective PPIs in patients with T2DM (PRIDE-2) study included 174 patients with a mean age of 51.5 years, of whom 59.8% were men. By the end of follow-up, the combination therapy produced marked improvements across major APD symptoms.

Relief was reported for the below with all outcomes achieving statistical significance.

  • Abdominal pain - 91.6%
  • Nausea - 89.5%
  • Loss of appetite - 93.6%
  • Altered bowel movements - 94.7%
  • Epigastric burning - 68.7%
  • Flatulence - 100.0%

The analysis revealed marked clinical improvements across all major APD manifestations. In parallel, favorable reductions were observed in key glycemic markers, including glycated hemoglobin , fasting blood sugar, postprandial blood sugar, and random blood glucose levels during the follow-up period. Safety findings further supported the use of the combination regimen. Only two participants reported mild adverse events, including muscle and bone pain, and no serious safety concerns were observed during the study period.

The findings demonstrated that the omeprazole–domperidone combination effectively addressed APD-related symptoms in patients with T2DM and maintained a favorable tolerability profile. The concurrent improvements observed in glycemic parameters also suggested a potential added clinical benefit, highlighting the need for larger prospective randomized studies to further validate these outcomes.
