Managing pain in older adults remains a significant clinical challenge, particularly as aging is associated with chronic illnesses and increased medication use. Nimesulide is widely used to treat a range of musculoskeletal and inflammatory conditions, but its safety profile continues to require careful evaluation in patients with complex healthcare needs.

The review sought to establish a clearer understanding of the clinical benefits and safety considerations associated with oral nimesulide in adults over 45 years by synthesizing evidence from diverse pain-related indications and major safety outcome measures. A systematic literature review was performed using PRISMA 2020 methodology, with evidence identified from PubMed and Google Scholar through August 2025. Randomized controlled studies, observational investigations, and pharmacovigilance reports were evaluated to characterize the efficacy, tolerability, and safety profile of oral nimesulide in middle-aged and older adults.

Analysis of 34 studies showed that nimesulide consistently provided effective pain relief and was generally well tolerated across a range of pain-related conditions in adults. Key findings included:

  • Effective management of osteoarthritis, low back pain, musculoskeletal disorders, and oral surgery-related pain.
  • Favorable gastrointestinal tolerability in controlled clinical studies with low rates of hepatic adverse events in interventional trials.
  • Pharmacovigilance data identified a rare but measurable risk of drug-induced liver injury, consistent with known nonsteroidal anti-inflammatory drugs (NSAID) class effects.
  • Renal and cardiovascular safety findings were generally similar to those observed with other commonly used NSAIDs.
  • Fixed-dose combinations containing paracetamol exhibited good overall tolerability.
  • Consistent analgesic efficacy, characterized by rapid onset of action and meaningful reductions in pain intensity.

Overall, the findings supported the short-term use of oral nimesulide in adults demonstrating effective pain relief and a generally favorable safety profile.
