Tackling postoperative and fracture-related pain in patients with type 2 diabetes (T2D) is challenging because these patients have heightened risk of cardiovascular, gastrointestinal, renal, and hepatic complications associated with nonsteroidal anti-inflammatory drugs (NSAIDs). Identifying an analgesic that delivers effective pain relief while minimizing systemic risks remains a clinical priority. This study explored whether lower-dose loxoprofen offers advantages over commonly prescribed NSAIDs.
This prospective cross-sectional study enrolled 174 T2D sufferers receiving treatment for bone fractures or undergoing orthopedic surgeries at orthopedic outpatient clinics. Participants underwent risk stratification for cardiovascular, gastrointestinal, renal, and hepatic complications. The study compared commonly prescribed NSAIDs for analgesic efficacy, dosing patterns, onset of action, peak analgesic effect, pain relief, nocturia, and overall safety during 4 weeks of follow-up.
During the 4-week follow-up, all evaluated NSAIDs reduced the numerical pain score (NPS), demonstrating their effectiveness in mitigating postoperative and fracture-related pain. Among the evaluated NSAIDs, loxoprofen was the only agent prescribed at a lower daily dose of 60–120 mg. Despite the lower dose, it attained the greatest reduction in NPS compared with the other NSAIDs, indicating superior analgesic potency.
Loxoprofen also illustrated the fastest onset of analgesic action and required the shortest time to reach peak analgesic effect among all NSAIDs evaluated. In addition, patients receiving loxoprofen experienced a reduction in nocturia and reported the strongest overall relief of postoperative and fracture-related pain. In comparison, celecoxib exhibited the highest variability in pain relief, suggesting less consistent analgesic performance than the other NSAIDs studied.
Among the NSAIDs evaluated, lower-dose loxoprofen demonstrated the most compelling combination of rapid analgesia, consistent pain control, and improved patient satisfaction. These findings position it as a valuable therapeutic option for easing postoperative and fracture-related pain in T2D, where balancing efficacy with safety is crucial.
