Maternal Lactobacillus reuteri SGL 01 supplementation significantly increases neonatal fecal Bifidobacterium, Lactobacillus, and Clostridium species without altering breast milk microbiota.

According to a recent study published in "Nutrients", maternal supplementation with Lactobacillus reuteri (L. reuteri) SGL 01 during lactation is associated with increased levels of selected bacterial taxa in the gut microbiota of exclusively breastfed neonates.

Early colonization of the infant gut by microorganisms is essential for immune system maturation and metabolic development. Although probiotics such as L. reuteri have been studied for their potential to impact infant microbiota, the effects of maternal probiotic supplementation during lactation remain insufficiently characterized. Hence, this study led by Elia Pagliarini et al. evaluated whether maternal supplementation with L. reuteri SGL 01 can alter the bacterial composition of breast milk and neonatal fecal microbiota during the first month of life.

This exploratory, prospective, randomized clinical trial enrolled lactating mothers of full-term, exclusively breastfed infants. Volunteers were divided into two groups:

  • 15 mother-infant pairs received L. reuteri SGL 01 at 1 × 10⁹ colony-forming units (CFU) once daily for 30 days.
  • 12 mother-infant pairs served as controls and received no supplementation.

Samples were procured at baseline and after 30 days. Researchers utilized real-time polymerase chain reaction to measure Bifidobacterium, Lactobacillus, Clostridium, and the Bacteroides fragilis group in breast milk and neonatal feces. A total of 27 mother-infant pairs completed the study, including 15 in the supplementation group and 12 in the control group.

  • Maternal supplementation did not markedly alter the composition of breast milk microbiota for any of the bacterial groups assessed.
  • In neonates whose mothers received L. reuteri SGL 01, fecal levels of:
    Bifidobacterium species significantly increased at day 30 (p < 0.001)
    Lactobacillus species significantly increased (p = 0.029)
    ○ Clostridium species significantly increased (p = 0.003)
  • No pivotal changes were detected in the neonatal microbiota of the control group, apart from a modest reduction in Clostridium species (p = 0.046).

To sum up, although 30 days of maternal L. reuteri SGL 01 supplementation had no measurable impact on the breast milk microbiota, it was linked with alterations in the neonatal fecal microbial profile. These findings indicate that maternal probiotics may affect infant gut colonization without directly reshaping the maternal milk microbiome.
