Long-term dulaglutide treatment sustains glycemic control and modest weight reduction beyond three years in real-world patients with long-standing type 2 diabetes.

Dulaglutide, a once-weekly glucagon-like peptide-1 receptor agonist (GLP-1 RA), has demonstrated efficacy in type 2 diabetes mellitus (T2DM), although evidence on its sustained real-world effects beyond three years has remained limited. A retrospective cohort study by Kim et al. evaluated whether glycemic and weight-related benefits persisted with prolonged dulaglutide therapy.

The study included 403 adults with T2DM who had continuously received weekly dulaglutide for at least three years between 2016 and 2023 at a tertiary hospital. The participants had a mean age of 60.0 years and an average diabetes duration of 17.4 years. Changes in glycated hemoglobin (HbA1c), fasting plasma glucose, and body weight were assessed over a mean follow-up of 4.3 years.

During follow-up, dulaglutide treatment was associated with durable improvements in glycemic control and body weight:

  • HbA1c decreased from 8.7% to 7.6%, representing a mean reduction of 1.1 % points (p < 0.001).
  • Body weight decreased by 2.9 kg on average (p < 0.001).
  • Baseline HbA1c was the strongest predictor of glycemic improvement.

For patients with long-standing T2DM, this real-world analysis suggests that dulaglutide's benefits may extend well beyond the initial treatment period—providing sustained improvements in glycemic control alongside modest weight reduction over more than 4 years.
