The burden of childhood obesity remains substantial. Nearly 80% of children with obesity continue to have obesity into adulthood, increasing their risk of suffering from type 2 diabetes mellitus (T2DM) and cardiovascular disease.
While lifestyle changes are often more effective in early life, individual responses vary wildly, creating a need for predictive tools to personalize treatment. Previous research indicated that identified genetic variants accounted for less than 8% of body weight variation. This led scientists to investigate the role of epigenetics as a connection between environmental factors and the genome.
Twenty-six prepubertal children, with an average age of 8 years, participated in a six-month Mediterranean-style dietary and exercise intervention. Their response to the lifestyle intervention was evaluated. The team examined whole-blood DNA methylation profiles to identify baseline markers that could distinguish "High Responders" from "Low Responders" before the intervention even began.

Researchers emphasized that these blood-based biomarkers remained stable throughout the six-month examination period. By identifying children who might respond poorly to standard lifestyle interventions, the findings offered a novel approach to early-stage obesity intervention.
