According to a retrospective cohort study, Levofloxacin-based regimen is better than Clarithromycin-based regimen for the management of Helicobacter pylori (H. pylori) infections. This study sought to determine eradication rates of Levofloxacin-based regimen and conventional first-line Clarithromycin regimen, as well as the effects of esophagogastroduodenoscopy (EGD) and diabetes on H. pylori elimination.

Patients infected with H. pylori who were treated for 10 or 14 days with Levofloxacin triple treatment or Clarithromycin-based regimen were included. A total of 245 subjects were randomized, 145 were given levofloxacin, and 100 were given Clarithromycin. When compared to Clarithromycin-based treatment, Levofloxacin-based treatment had a greater eradication rate (62% versus 74.5%). The 14-day and 10-day Levofloxacin-based treatment had the greatest H. pylori elimination rate when compared to the 14-day and 10-day Clarithromycin-based treatment, as shown in Table 1:

H. pylori eradication was unaffected by diabetes or EGD results. The findings of this study justify usage of Levofloxacin-based regimen for 14 days as first-line therapy for H. pylori infection, irrespective of EGD findings or diabetes.
