Low levels of gluten trigger measurable immune activation in treated celiac disease, even without noticeable symptoms.
Could gluten be harming patients with celiac disease even when they feel perfectly well? A new randomized, double-blind, placebo-controlled study suggests the answer is yes.
Celiac disease results from an immune reaction to gluten, but the threshold at which very small gluten exposures activate the immune system remains uncertain, shaping current debates on gluten-free labeling standards. The investigators therefore determined the lowest gluten dose that induced a measurable interleukin-2 (IL-2) response and associated symptoms in treated celiac disease.
A randomized dose–response study was performed in 51 adults suffering from biopsy-proven celiac disease who had adhered to a gluten-free diet for over 2 years. Participants underwent three oral gluten or placebo challenges ranging from 1 to 1000 mg at 4-week intervals. The study primarily assessed a ≥2-fold rise in serum IL-2 within 6 hours, with symptom reporting evaluated as a secondary outcome.
Immune response thresholds were determined using interval-censored survival analysis. All 51 participants successfully completed 153 gluten or placebo challenges. Gluten triggered a dose-dependent increase in serum IL-2 levels, indicating early immune activation. A ≥2-fold rise in IL-2 was observed in:
No IL-2 response was detected following 5 mg, 2 mg, 1 mg, or placebo challenges. Modeling further showed that the estimated median effective dose (ED50) was 111 mg, while the ED10, ED05, and ED01 were 2.4 mg, 0.8 mg, and 0.1 mg, respectively. These findings indicate that immune activation may commence at submilligram gluten exposures in the most sensitive individuals. Patient-reported symptoms increased after gluten challenges but remained comparable to placebo, indicating that symptoms did not reliably reflect early immune activation.
The research challenged the long-held belief that "no symptoms" means "no harm." By demonstrating immune activation at trace gluten doses, the study paved the way for evidence-based updates to gluten-labeling standards and more precise protection for those suffering from celiac disease.
