Background

In-office dental bleaching is widely performed to improve tooth color and meet the growing demand for aesthetic dental care. However, whether repeated applications of high-concentration hydrogen peroxide provide additional clinical benefits over a single application remains uncertain. To clarify this, researchers evaluated the comparative effects of single- and multiple-application bleaching protocols on tooth sensitivity, whitening outcomes, and one-month color stability in a randomized controlled trial (RCT).

Method

Overall, 64 participants with moderate tooth discoloration were enrolled. Using a split-mouth design, each participant received both single- and multiple-application bleaching protocols with neutral high-concentration hydrogen peroxide gels (40% Opalescence Boost and 37.5% Pola Office Plus) over two treatment sessions.

Tooth sensitivity was assessed during and after treatment via the visual analog scale (VAS) and numerical rating scale (NRS). Bleaching efficacy was checked using the VITA Easyshade spectrophotometer. This was measured via ΔEab, ΔE00, and Whiteness Index for Dentistry (WID), along with Vita Classical Shade Guide Units (SGU) at baseline, 7 days, and 1 month after bleaching.

Result

The study demonstrated that reducing the number of bleaching gel applications did not compromise whitening outcomes or patient comfort. Key findings included:

  • Comparable tooth sensitivity: Paired sensitivity outcomes were observed in 100% of participants treated with Pola Office Plus and 93.3% receiving Opalescence Boost, with no vital differences in VAS or NRS scores between the single- and multiple-application protocols (p > 0.05).
  • Enhanced early whitening: The single-application protocol achieved greater improvements in ΔE00 and ΔWID at 7 days than the conventional protocol (p < 0.05).
  • Equivalent long-term efficacy: By one month, objective color measurements (ΔEab, ΔE00, and ΔWID) were comparable between both approaches (p > 0.05).
  • Consistent shade improvement: ΔSGU remained similar throughout follow-up, with no significant differences detected between protocols.

Overall, the findings indicated that a simplified single-application bleaching protocol achieved clinical outcomes comparable to the conventional multi-application approach while offering a more efficient treatment strategy.

Conclusion

By demonstrating comparable whitening outcomes with fewer treatment steps, single-application bleaching could redefine routine in-office whitening protocols. The approach combined clinical effectiveness with procedural simplicity, offering benefits for both clinicians and patients.
