Background

Cardiovascular disease (CVD) continues to pose a major global health challenge, prompting increasing interest in non-traditional risk factors beyond hypertension, diabetes, and dyslipidemia. Chronic Helicobacter pylori (H. pylori) infection has recently been implicated in CVD through its potential role in promoting systemic inflammation and metabolic disturbances. Researchers assessed whether successful eradication of H. pylori resulted in long-term improvements in lipid metabolism, insulin resistance, and other biomarkers associated with cardiovascular (CV) risk.

Method

In this randomized trial, 72 adults with H. pylori infection were randomly assigned to the following 14-day eradication therapies:

  • Group 1: Esomeprazole, amoxicillin, clarithromycin
  • Group 2: Esomeprazole, amoxicillin, metronidazole, colloidal bismuth subcitrate

Participants were monitored from baseline through one year after successful eradication, with serial evaluation of lipid metabolism, lipoprotein subfractions, homeostatic model assessment of insulin resistance (HOMA-IR), and trimethylamine N-oxide (TMAO), enabling investigators to assess the long-term cardiometabolic impact of infection clearance.

Result

Among the randomized participants, 13.9% (10/72) were unavailable for follow-up at one year. Remarkable improvements were observed in lipid-related biomarkers, whereas anthropometric and glycemic measures remained stable.

  • Total cholesterol: Decreased significantly (p=0.003)
  • Low-density lipoprotein (LDL) cholesterol: Decreased significantly (p=0.010)
  • Small dense LDL particles: Reduced significantly (p=0.037)
  • Urinary TMAO: Marginal reduction (p=0.048)
  • Body mass index (BMI): Unchanged (p=0.799)
  • Waist circumference: Unchanged (p=0.305)
  • HOMA-IR: No significant difference (p=0.275)

The overall findings pointed to favorable changes in lipid metabolism and a modest decline in TMAO concentrations, while insulin resistance and body composition remained unaffected during the one-year follow-up.

Conclusion

The results expanded the clinical relevance of H. pylori eradication by suggesting potential benefits beyond gastric disease management. While confirmation in larger populations remains necessary, the study provided important evidence supporting the role of chronic infection in shaping CV risk profiles.
