Persistent heartburn despite PPIs? 5 unusual conditions may be responsible!

What's new?

Five conditions—achalasia, eosinophilic oesophagitis, belching and rumination disorders, gastroparesis, and Zollinger–Ellison syndrome—can mimic refractory GERD, making targeted diagnostic testing essential when PPI therapy fails.

In modern gastroenterology, persistent reflux-like symptoms despite optimized proton pump inhibitor (PPI) therapy present a recurring clinical dilemma. While treatment failure is frequently presumed to stem from unmanaged acid exposure, mounting clinical evidence indicates that alternative or overlapping non-acid pathologies are often the primary drivers of symptom persistence.

A review of selected literature indexed in PubMed highlights five major gastroesophageal reflux disease (GERD) mimickers that demand specialized diagnostic toolkits rather than a heavier prescription pad:

  • Achalasia & esophageal motility disorders: When the lower esophageal sphincter fails to relax or peristalsis breaks down, trapped pressure mimics severe reflux. High-resolution manometry is the gold standard for unmasking this motor failure.
  • Eosinophilic esophagitis: This immune-mediated, allergen-driven inflammatory ailment mimics acid damage with refractory heartburn and dysphagia. Diagnosis requires targeted esophageal mucosal biopsies during endoscopy.
  • Behavioral belching & rumination disorders: Unconscious supragastric air swallowing or effortless postprandial regurgitation can masquerade as treatment-resistant GERD. Distinguishing these requires multichannel intraluminal impedance-pH monitoring.
  • Gastroparesis: Delayed gastric emptying creates a mechanical pressure cooker in the stomach, driving reflux symptoms. Pinpointing this delay relies on gastric emptying studies.
  • Zollinger-Ellison syndrome (ZES): It is defined as a rare neuroendocrine tumor (gastrinoma) driving severe, intractable acid hypersecretion. Catching this requires fasting serum gastrin measurements and gastric acid analysis.

Persistent reflux symptoms in the face of optimized PPI therapy must never trigger a default escalation of acid suppression. By moving beyond a one-size-fits-all approach and deploying advanced physiologic, endoscopic, and manometric testing, clinicians can finally decode the true root cause—sparing patients unnecessary procedures and aligning intervention directly with the underlying pathophysiology.
