Adding tirzepatide to ixekizumab boosts skin clearance and weight loss in psoriasis

What's new?

Combining ixekizumab with tirzepatide improves complete skin clearance and weight reduction compared with ixekizumab alone in adults with psoriasis and overweight or obesity.

For decades, dermatologists have faced a persistent bottleneck: up to 78% of people with psoriasis also struggle with overweight or obesity, a metabolic overlap that not only worsens skin inflammation but actively blunts the performance of targeted biologics. Now, the outcomes of a pivotal phase 3b clinical trial reveal a dual-target strategy that tackles both diseases head-on.

By pairing the interleukin-17A inhibitor ixekizumab with the dual glucose-dependent insulinotropic polypeptide/glucagon-like peptide-1 (GIP/GLP-1) receptor agonist tirzepatide, researchers demonstrated that simultaneous immune suppression and metabolic resetting can attain outcomes that standard monotherapy simply cannot match. The 52-week, open-label, randomised clinical trial enrolled 274 adults across 72 sites in the United States. Subjects were assigned in a 1:1 ratio to receive:

  • Combination group: Ixekizumab + tirzepatide
  • Monotherapy group: Ixekizumab alone

Both groups also received diet and exercise interventions. The primary endpoint was the simultaneous achievement of 100% improvement in the Psoriasis Area and Severity Index (PASI 100) and at least 10% weight reduction at week 36. Among the participants, 231 (84.3%) completed treatment through week 36. The key findings were:

  • Combined skin clearance and weight loss: 27.1% of participants receiving ixekizumab + tirzepatide achieved PASI 100 alongside at least 10% weight reduction, compared with 5.8% receiving ixekizumab alone.
  • Complete skin clearance: PASI 100 was attained by 40.6% vs. 29% of participants, respectively.
  • Skin improvement and weight loss: 79.9% vs. 17.9% simultaneously achieved PASI 75 (at least 75% improvement in PASI score) and at least 5% weight reduction.
  • Weight reduction: At least 10% weight loss was attained by 69.2% of participants receiving combination therapy versus 9.1% receiving ixekizumab alone.

The reported adverse events largely aligned with the known safety profiles of both medications, with no novel safety signals identified. Gastrointestinal events occurred more frequently with combination therapy. The most commonly noted events included:

  • Nausea: 24.6% vs. 2.2%
  • Diarrhea: 14.5% vs. 1.5%
  • Constipation: 13.0% vs. none
  • Vomiting: 8.7% vs. 1.5%

Injection-site reactions occurred at similar rates in both groups (12.3% vs. 12.7%). No deaths were reported. The findings highlight the potential value of addressing obesity alongside psoriasis rather than focusing exclusively on skin manifestations. Combining ixekizumab with tirzepatide improved complete skin clearance, promoted substantial weight loss, and offered additional metabolic benefits. Hence, this combination appears to offer a promising approach for adults with moderate-to-severe plaque psoriasis and overweight or obesity.
