Maintaining periodontal stability depends not only on professional treatment but also on effective daily control of dental biofilm between dental visits.
Home-applied dual-light photodynamic therapy improves bleeding on probing, visible plaque, and periodontal pocket outcomes when added to supportive periodontal care in patients with stage I–III periodontitis.
Maintaining periodontal stability depends not only on professional treatment but also on effective daily control of dental biofilm between dental visits. Although light-based therapies have illustrated antibacterial activity, evidence for their regular home application during periodontal maintenance has been limited. The HOPE-CP study evaluated whether adding home-applied dual-light therapy to supportive periodontal care (SPC) could improve clinical periodontal outcomes.
This 6-month randomized controlled trial was conducted in Finland. Of 511 subjects assessed for eligibility, 200 were randomly allocated 1:1 to receive either:
Both groups received standardized oral hygiene instructions and an electric toothbrush. All the subjects underwent baseline anti-infective therapy according to a standardized protocol, with debridement above and below the gingival margin as indicated. Assessments were performed at baseline, 3 months, and 6 months for bleeding on probing (BOP, the primary endpoint), visible plaque index (VPI), and sites with periodontal probing depth (PPD) ≥4 mm, with changes analyzed using generalized estimating equations and linear mixed-effects models.
At baseline, the treatment groups were comparable with respect to BOP, VPI, and periodontal pocket depth. After 3 and 6 months, adding home-applied dual-light therapy produced better periodontal parameters than SPC alone (Table 1).

Regular, daily home application of dual-light PDT enhanced clinical outcomes during supportive periodontal maintenance. Incorporating this light-based antibacterial and photobiomodulatory device into daily home-care regimens offered a safe, non-invasive, and effective strategy to suppress biofilm, alleviate periodontal tissue inflammation, and prevent disease recurrence between professional dental visits.
