Local drug delivery using i-PRF, oxygen-releasing gel, and hyaluronic acid gel improves periodontal health. Oxygen-releasing gel shows the greatest clinical benefit, while i-PRF demonstrates superior antimicrobial activity.
The search for more effective adjuncts to conventional periodontal therapy continues as clinicians aim to achieve better pocket healing while limiting disease recurrence. In a split-mouth randomized controlled trial (RCT), three biologically distinct local drug delivery (LDD) systems were evaluated to determine whether they offer comparable or complementary benefits when added to routine scaling and root planing (SRP).
The study enrolled 12 patients with a total of 80 periodontal pockets measuring 3–6 mm in probing depth. Each periodontal pocket was randomly allocated to one of four treatment groups: SRP alone, SRP + injectable platelet-rich fibrin (i-PRF), SRP + oxygen-releasing gel, or SRP + hyaluronic acid (HA) gel.
Periodontal health was monitored using standard clinical indices together with dark-field microscopic assessment of spirochete counts over a 3-month follow-up, providing insight into both tissue healing and microbial control. Every adjunctive therapy contributed to measurable improvements in periodontal health, with significant gains observed across plaque accumulation, gingival inflammation, pocket depth, attachment level, and gingival recession compared with baseline.
Although the overall clinical outcomes were comparable among treatment groups, each adjunct demonstrated a distinct therapeutic strength. Oxygen-releasing gel produced the most pronounced clinical improvements, whereas i-PRF achieved the greatest reduction in spirochete counts, indicating superior antimicrobial activity. HA gel also enhanced treatment outcomes, reinforcing its role as a useful adjunct to nonsurgical periodontal therapy.
Thus, different LDD systems may address different therapeutic goals. Oxygen-releasing gel appears particularly suited for maximizing clinical periodontal improvement, while i-PRF may be advantageous when microbial reduction is the primary objective. HA gel also contributes meaningful benefits, supporting a more individualized approach to adjunctive periodontal treatment based on patient needs and treatment priorities.
