Obesity acts as a critical disease catalyst in pediatric patients, doubling T1DM risk and necessitating a shift towards weight-centric metabolic care models.
The analysis of 67 studies published between 2010 and 2026 evaluated the complex bidirectional relationship between obesity and type 1 diabetes mellitus (T1DM) in children and adolescents. The collective findings supported the "accelerator hypothesis", which proposed that weight-related insulin resistance increased metabolic stress on pancreatic beta-cells. This, in turn, accelerated autoimmune destruction and triggered earlier clinical manifestation in genetically susceptible individuals.
Population-based data from a cohort of 1.4 million adolescents demonstrated that obesity was associated with a two fold increased risk of incident disease (hazard ratio [HR] 2.05). Furthermore, Mendelian randomization provided causal evidence for this link, revealing that a genetically predicted 1-standard-deviation increase in childhood body mass index (BMI) was associated with a 32% higher risk of developing T1DM.
