Cochlear implantation stands as one of the most transformative surgical interventions in pediatric audiology, offering children with severe-to-profound sensorineural hearing loss a viable pathway to spoken language development and social integration. As implantation rates rise globally and candidacy criteria continue to expand toward younger and more complex patients, understanding the real-world safety landscape of this procedure becomes increasingly critical.

While short-term surgical outcomes are well-documented in smaller series, large-scale, long-term complication data from high-volume centers remain relatively scarce, leaving clinicians without a robust benchmark for counseling families and refining perioperative protocols. Against this backdrop, investigators set out to comprehensively characterize the incidence, spectrum, timing, and clinical management of post-operative complications across a decade of pediatric cochlear implantations at a single tertiary referral center.

A retrospective cohort study was carried out, enrolling all pediatric patients aged 18 years or younger who underwent primary cochlear implantation at a single tertiary center. Of 507 initially assessed, 493 children met the inclusion criteria and formed the final study cohort, with 93.7% maintaining a minimum follow-up of 12 months.

Complications were systematically classified by onset, early (within 30 days of surgery) or late (beyond 30 days), and further categorized by type into infection, skin-related events, device failure, trauma, and electrode misplacement. Descriptive statistics were applied for data assessment, and management strategies corresponding to each complication type were comprehensively documented.

Across a decade and nearly 500 implantations, the overall complication burden proved reassuringly low. Key findings included:

  • Overall complication rate was 4.3%, with just 21 events across 493 children.
  • Late complications predominated, accounting for 66.7% of all events (14 of 21), underscoring the need for sustained long-term follow-up beyond the immediate post-operative window (Table 1).

  • Re-implantation was required in only 1.4% of the cohort, exclusively driven by device failure cases.
  • No single complication type was found to be unmanageable under standardized clinical protocols.

This decade-long, single-center experience reinforced that pediatric cochlear implantation carries an exceptionally favorable safety profile when performed within a structured, high-volume surgical environment. While complications are infrequent, their predominantly late onset demands vigilant, long-term post-operative surveillance rather than reassurance at discharge alone.
