Combined lifestyle therapy and GLP-1 RAs emerge as a high-impact, evidence-backed strategy delivering markedly superior weight loss and cardiometabolic gains in adults with overweight or obesity.


 

Lifestyle modification continues to serve as the cornerstone of obesity management, while glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are widely recognized for their potent metabolic effects. However, the true extent of their combined benefit has remained an unresolved clinical question. Seeking to clarify this gap, the researchers set out to investigate whether coupling GLP-1–based pharmacotherapy with structured lifestyle interventions could unlock additive or even synergistic advantages beyond those achieved through lifestyle change alone.

The research team conducted a rigorous systematic review and meta-analysis of randomized controlled trials identified through PubMed, Embase, and the Cochrane Library. Eligible trials included adults with overweight or obesity who participated in structured lifestyle programs and were randomly assigned to receive either a GLP-1 RAs or a placebo. Researchers evaluated a wide range of clinical outcomes, including body weight, anthropometric measures, blood pressure, glycemic indices, and lipid parameters. Lifestyle modification + GLP-1 RAs resulted in significantly greater weight reduction and metabolic improvements across most evaluated outcomes, with the exception of HDL-C (Table 1).

Subgroup analyses revealed that longer treatment duration, weekly dosing regimens, and use of newer GLP-1 RAs, particularly semaglutide and tirzepatide, were related to more pronounced weight loss. Studies conducted in North America also demonstrated stronger effects, suggesting potential influences of healthcare delivery models, adherence, or baseline population characteristics.

In summary, combining lifestyle modification with GLP-1 weight-loss medications delivers clinically meaningful weight loss and improves multiple cardiovascular and metabolic biomarkers, supporting a multimodal approach to obesity treatment—while highlighting the need for individualized, long-term management strategies.
