Tapinarof 1% cream used once daily improves disease severity and pruritus in mild-to-severe atopic dermatitis, achieving nearly threefold higher clinical response rates with an acceptable safety profile.
Tapinarof 1% cream administered once per day remarkably improves disease severity and pruritus in atopic dermatitis (AD), according to the outcomes of a systematic review and meta-analysis of randomized controlled trials.
Conducted in line with PRISMA guidelines, the analysis synthesized evidence from placebo-controlled studies identified via comprehensive searches of PubMed, Embase, the Cochrane Library, and ClinicalTrials.gov. Notably, 4 publications encompassing 5 randomized trials were included, representing a total of 1,191 patients with mild-to-severe AD. The primary efficacy outcomes were investigator global assessment (IGA) success and achievement of at least a 75% betterment in the eczema area and severity index (EASI-75).
Secondary endpoints assessed pruritus reduction using the peak pruritus numeric rating scale (PP-NRS) and the occurrence of adverse events. At 8 weeks, patients treated with tapinarof 1% cream once daily were more than 3 times as likely to achieve IGA success compared with placebo (risk ratio [RR] 3.03). Similarly, EASI-75 response rates were nearly threefold higher in the tapinarof group (RR 2.99). Clinically meaningful itch relief was also noted.
Tapinarof treatment substantially increased the likelihood of achieving a ≥4-point improvement in PP-NRS scores, a threshold considered clinically important (RR 1.58). In terms of safety, tapinarof was generally well-tolerated. Although a modest rise in overall adverse events was noted compared with placebo (RR 1.39), the incidence of serious adverse events did not differ considerably between treatment groups (RR 1.98). Overall, these findings reinforce tapinarof 1% cream as an effective and well-tolerated non-steroidal topical therapy for AD, offering meaningful improvements in both clinical severity and pruritus across a broad patient population.
