Progestogens and COCPs offer comparable relief in endometriosis

Progestogens and combined oral contraceptive pills show equivalent effectiveness and tolerability for symptom relief in endometriosis.

A systematic review and meta-analysis has found no significant difference between progestogens and combined oral contraceptive pills (COCPs) in easing endometriosis-related pain and other symptoms. The study, which included 7 trials with 948 participants (476 of whom received progestogens), assessed the effects of these treatments on pelvic pain, dysmenorrhea, dyspareunia, and psychological health, along with their side effect profiles.

Researchers scoured major databases, including Cochrane Central Register of Controlled Trials, Embase, and PubMed. Only studies directly comparing progestogens and COCPs for endometriosis-related pain were included. Abstracts, case reports, opinion pieces, and studies without meaningful results were eliminated. No prominent differences were reported between progestogens and COCPs in mitigating pelvic pain, dysmenorrhea, dyspareunia, or psychological health (Table 1).

Likewise, both treatments exhibited comparable side effect profiles, with no notable disparities in breast tenderness, weight gain, amenorrhea, or bleeding. The findings suggest that both progestogens and COCPs offer similar relief for endometriosis-related pain and associated symptoms. Given their comparable effectiveness and side effect profiles, treatment choice can be guided by patient preferences, tolerability, and cost considerations.
