As women transition through the perimenopausal period, selecting a contraceptive option that balances efficacy with cardiovascular and metabolic safety remains an important clinical consideration. NOMAC-E2 has emerged as an alternative combined oral contraceptive(COC), but comparative real-world evidence in women remains limited. This real- world study aimed to evaluate the safety and contraceptive effectiveness of NOMAC-E2 compared with levonorgestrel-containing COC (COCLNG) in women aged over 40 years.
A multinational observational cohort study followed women initiating NOMAC-E2 or COCLNG in routine clinical practice across three continents. Prospective questionnaire-based follow-up was used to evaluate thromboembolic safety, contraceptive effectiveness, and treatment-related changes in mood and body weight.
A total of 7,762 women receiving NOMAC-E2 and 6,059 women using COCLNG were included in the analysis.

The findings indicated that NOMAC-E2 achieved contraceptive efficacy and safety outcomes comparable to those observed with COCLNG in women over 40 years of age. The study supported its use as a reliable contraceptive alternative for perimenopausal women, with no apparent increase in thromboembolic risk or adverse effects on mood and body weight.
