Background

Effective control of post-endodontic pain, especially after root canal treatment, is fundamental for improving patient comfort and clinical outcomes. While ketorolac, a potent nonsteroidal anti-inflammatory drug (NSAID), is widely used for dental pain relief, its comparative efficacy against other analgesics remains unclear. Hence, this systematic review and meta-analysis explored the potency of ketorolac versus alternative pain medications. 

Method

A comprehensive literature search was executed across major databases, including PubMed, Scopus, Embase, Web of Science, Cochrane Library, ScienceDirect, SinoMed, CNKI, Wanfang Data, and Webvpn. Continuous outcomes were analyzed via the inverse variance method with a random-effects model, while dichotomous data were assessed via risk ratio and the Mantel–Haenszel fixed-effect model. The standardized mean difference was employed as the primary effect measure.

Result

Out of 386 identified randomized clinical trials, 7 fulfilled the criteria for qualitative synthesis, and 5 were incorporated in the quantitative meta-analysis. Most studies (5/7) favored ketorolac for post-endodontic pain relief, while 1 reported comparable efficiency and another found it less effective than alternatives. Meta-analysis revealed that ketorolac yields considerably superior pain relief at 6 hours post-treatment compared to other analgesics. However, no prominent differences were noted at 12 and 24 hours. Notably, those treated with ketorolac exhibited a reduced need for rescue analgesics, highlighting its sustained early analgesic benefit.

Conclusion

Ketorolac proved highly beneficial for early post-endodontic pain relief, particularly within the first 6 hours after root canal therapy. Its ability to decrease additional analgesic requirements supported its role as a preferred pain management option in endodontics, enhancing patient comfort, and recovery.
