For millions living with fibromyalgia, persistent pain is a daily battle. But what if a simple, structured aerobic exercise routine could offer relief?

A novel review published in the European Journal of Pain suggested the implementation of an individualized exercise plan—according to the frequency, intensity, and type for maximum pain relief.

David Casanova-Rodríguez and researchers analyzed 17 randomized clinical trials sourced from PubMed, Web of Science, Scopus and PEDro, the Physiotherapy Evidence Database, to determine how different aerobic regimens impact pain relief in fibromyalgia patients. Using the FITT-VP model (Frequency, Intensity, Type, Time, Volume, and Progression), they examined a range of exercise interventions, including:

  • Music-based workouts
  • Interval training
  • Pool exercises
  • Cycling, swimming and walking

The studies varied widely, with exercise frequency ranging from 1 to 10 sessions per week, intensity from light to vigorous, and program durations spanning 3 to 24 weeks. The findings strongly supported aerobic exercise as a clinically effective strategy for fibromyalgia pain relief. On average, patients who swear by aerobic programs had a considerable reduction in pain (mean difference  −0.49; confidence interval [−0.90, −0.08]; p = 0.02).

The overall risk of bias varied, with 6 studies rated low risk, 5 with some concerns, and 6 classified as high risk. In a nutshell, these outcomes strengthen the importance of personalized exercise prescriptions for fibromyalgia patients.
