The β-D-mannuronic acid as a new, highly safe and efficient drug in the treatment of rheumatoid arthritis (RA) as elucidated from a multinational, phase III clinical study published in 'Inflammopharmacology' Earlier phase I/II study has described that the oral administration of drug- β-D-mannuronic acid (M2000) showed a significant therapeutic effect in RA patients. The safety and efficacy of this drug in RA patients have been evaluated in this phase III clinical trial.

Endoscopic sinus surgery (ESS) is helpful for the treatment of chronic rhinosinusitis refractory to the conservative measures. In this trial, used as an adjunct to standard perioperative methods to reduce blood loss, the total intravenous anesthesia (TIVA) in comparison with volatile anesthetic improves intraoperative visualization during ESS.

Introduction:

Bleeding during endoscopic sinus surgery (ESS) can impede visualization and delay the surgical progress. The role that the anesthetic technique may have on the quality of the surgical field during ESS has been previously studied. But, the meta-analyses have considered the current literature inconclusive and lacking methodological consistency. This study was devised keeping these critiques in mind to assess the effect of TIVA versus inhaled anesthetic on the quality of surgical field during ESS.

Key Take-Away:

This study reported that tofacitinib 10mg + MTX and baricitinib 4mg + MTX were the most efficacious interventions for rheumatoid arthritis patients with inappropriate response to disease-modifying anti-rheumatic drugs (DMARDs) or biologics therapy. Also, they were not associated with a significant risk of SAEs. But, long-term studies are awaited in a large number of patients with active RA that is inadequately responsive to MTX or biologics to determine the relative efficacy and safety of tofacitinib and baricitinib.

This study, based on an international survey emphasis on the difficulty in establishing an unambiguous concept of difficult-to-treat rheumatoid arthritis (RA), which is seen as a heterogeneous condition and not fully covered by current EULAR recommendations. A more detailed survey is still needed to study the management of difficult-to-treat RA further and the recently established EULAR Task Force will explore the management of difficult-to-treat RA.

This retrospective case series by Guo C et al. reported that supramalleolar osteotomy with intra-articular osteotomy had relatively good clinical results in the treatment of malunited medial impacted ankle fractures for maintaining ankle function. This is the right choice for the treatment of malunited medial impacted ankle fractures related to varus deformity. But, the surgeons should be aware of the loss of ankle motion post-operatively in most patients.

Introduction:

Malunion of a medial impacted ankle fracture may lead to varus ankle deformity. The use of supramalleolar osteotomy plus intra-articular osteotomy in patients with malunited medial impacted ankle fractures was explored in this study.
