A study suggests not to prescribe antibiotic prophylaxis among the patients undergoing operative hysteroscopy

A recent study published by Muzii L and colleagues suggests avoiding prescribing routine antibiotic prophylaxis before operative hysteroscopy.

Antibiotic prophylaxis is a process that involves the use of antibiotics before any surgery to prevent microbial infections. This is known to reduce microorganism's colonization pressure introduced at the time of operation to a level that the patient's immune system can overcome the infection. Like all other surgeries, hysteroscopy also involves a risk of having infectious complications followed by the procedure, which can lead to morbidity and potential mortality.

A multicentric randomized controlled trial was conducted to find out the effect of antibiotic prophylaxis administration on the incidence of infectious complications among patients undergoing operative hysteroscopy.

This trial was conducted between January 2012 and December 2013.  Females having myomas, endometrial polyps or endometrial hyperplasia and undergoing operative hysteroscopy were chosen for the study. The participants were divided into two groups. Both groups received 2 g of cefazolin intravenously. One group received antibiotic before the procedure and other under no treatment.

The results found no statistical difference in case of endometritis, pain, postoperative fever, cervicitis-vaginitis, pelvic inflammatory disease, pelvic abscess, and bleeding. Moreover, no statistical distinction was seen among side effects attributable to antibiotic prophylaxis like vomiting, nausea, allergy, cephalea, diarrhea, dizziness and meteorism. Overall, the study findings recommended not to prescribe routine antibiotic prophylaxis before operative hysteroscopy.
