Segmental Resection and Disc resection Improves Fertility and Reduces Pain in Women with Deep Infiltrating Endometriosis

Surgery with either disc resection or segmental resection exerts similar efficacy in reducing pain and improving fertility among women with deep infiltrating endometriosis (DIE), confirms a new study published in Obstetrics and Gynaecology Journal.

Gernot Hudelist and colleagues conducted a prospective cohort study to assess the surgical effects of both disc and segmental resection (DR and SR) on fertility, quality of life and pain related to colorectal DIE. The analysis involved 134 patients diagnosed with symptomatic DIE of rectosigmoid up to 25cm from the anal verge from March 2011 to December 2016.

Out of the total 134 patients, DR was performed in 32 patients and SR was performed in 102 patients. The associated complications rates were measured as per the Clavien‐Dindo classification.

No significant difference was observed regarding pain reduction, functional and fertility outcomes between the groups. Both groups showed a considerable increase in quality of life and reduction in endometriosis pain. The mean hospital stay, the discrepancy in hemoglobin levels, complication rates and duration of surgery was similar for both groups.

A total of 112 patients were followed-up for the long term. Overall pregnancy rate observed after the treatment was found to be 63.4%. Out of total 61 infertile women, 26 got pregnant immediately and 13 through in vitro fertilization. Only eight patients without any significant statistical difference between the groups showed some complications. The study outcomes reflect no difference in the efficacy of SR or DR in reducing endometriosis-related pain and fertility complications.
