Percutaneous Kyphoplasty for Vertebral Compression Fractures

As per a recent study conducted by Zhe Li and colleagues, unilateral percutaneous kyphoplasty (PKP) represents a reliable and safe therapy for osteoporotic vertebral compression fractures (OVCFs) with an intravertebral cleft (IVC). The study involved 309 patients, 266 females and 43 males who obtained PKP treatment from 2010 to 2016. Based on the pre-operative magnetic resonance imaging, the patients were categorized into two groups; OVCFs with IVC and OVCFs without IVC. The posterior wall height (PWH), kyphotic angle (KA) and anterior wall height (AWH) before, after the treatment and at final follow-up were assessed in both groups.

The follow up was done for about 12~34 months with an average of 16.2 months. The degree of IVC was linked to older age and lower bone mineral density (BMD). Both groups exhibited immediate significant improvement in PWH, KA, and AWH after the surgery. The improvement in Oswestry disability index (ODI) and Visual analogue scale (VAS) was also seen among both the groups without any difference. A significant enhancement in KA and reduction in AWH was observed in both groups at the final follow-up. However, the group having no IVC showed decreased KA as compared to the group with IVC after one year of surgery. The IVC group exhibited larger and subsequent volume of bone cement. The incidences of bone cement leakage were higher in the IVC group; however, no statistical difference was observed among the groups.

The PKP showed excellent efficacy and safety in managing OVCFs with IVC. However, the IVC group exhibited a higher percentage of KA rebound and bone cement leakage. The leakage can by injecting the cement carefully and repetitiously. In the case of cement leakage, the operation should be stopped immediately. More extended rehabilitation interventions like wearing an appropriate brace, doing exercise to strengthen low-back muscle, and substituting bending with squatting are necessary to prevent KA rebound in patients with OVCFs with IVC.
