Fibromyalgia can cause changes in bone mineral density

According to the previous studies, fibromyalgia syndrome (FMS) is associated with low level of physical activity and exercise, which may cause an increased risk of osteoporosis. However, recent studies of bone mineral density (BMD) in fibromyalgia have depicted contradictory results.

For a better understanding of the relationship between FMS and BMD, Upala S et al. conducted a systematic review and meta-analysis. The database searching of MEDLINE and EMBASE was done from inception through May 2016. As per the inclusion criteria in adult patients, the association between FMS and BMD was observed The diagnosis of the FMS was confirmed as per the  American College of Rheumatology criteria. The dual-energy X-ray absorptiometry was used to measure the BMD of the lumbar spine and femoral neck. A random effect, generic inverse variance method was used to calculate the pooled mean difference (MD) of BMD at each site and 95% confidence interval (CI). The Q statistic and I2 was used to quantify the between-study heterogeneity of effect size.

A total of 680 patients in four observational studies was used to extract the data. The BMD is notably decreased in patients with FMS compared with controls with pooled MD of −0.02 (95% CI −0.03 to −0.01, p = 0.003, I2 = 0%) at the lumbar spine (L2–L4).  The BMD was not notably decreased in patients with FMS compared with controls with pooled MD of 0.01 (95% CI −0.02 to 0.01, p = 0.23, I2 = 0%) at femoral neck.

Overall, the BMD at the lumbar spine was decreased in FMS compared with normal individuals this meta-analysis. To check the risk of osteoporosis, the patients with FMS needs to be investigated.
