Analgesic Capability of Preoperative Pregabalin, Magnesium Sulphate and their Combination in Acute Post-thoracotomy Pain

The combined preoperative single dose of Pregabalin and magnesium sulphate is an effective method for easing the postoperative pain and total morphine consumption in patients undergoing thoracotomy as revealed in a study published in 'The Clinical Journal of Pain'.

Ahmed Salah Ahmed et al. examined the effects of the preoperative combination of oral pregabalin and intravenous magnesium sulphate as analgesic adjuvants in the post-thoracotomy pain.

A total of 120 patients with ASA physical status II were designated randomly into one of the four groups. Group MP was given 300mg pregabalin orally and an intravenous (IV) infusion of magnesium sulphate 50mg/kg mixed with 200mL normal saline (NS). Group P was given 300mg pregabalin orally and 200mL NS infusion.

Group M was given an IV infusion of magnesium sulphate 50mg/kg mixed with 200mL NS and a placebo capsule. Group C was given placebo capsule and an IV infusion of 200mL NS. In all the groups, all the medications were given 1 hour before the surgery. In the first 24 hours postoperatively, total morphine consumption and visual analogue scale (0-10 VAS) were used as a pain measurement tool, and postoperative nausea and vomiting (PONV) were also examined.

In the first 24 hrs postoperatively, total morphine consumption decreased significantly in group MP (28.47±5.76mg) as opposed to group P (33.97±6.34mg), group M (40.87±4.4mg) and group C (42.2±6.1mg). Also, all the patients were on the patient-controlled analgesia (PCA); the VAS scores were in the accepted range in the four groups. But, there was a statistically significant difference at 0 and 4 hours postoperatively favouring the groups MP and P. PONV decreased significantly in groups MP, P and M in comparison to group C.
