Lower serum clusterin levels in patients with erosive hand osteoarthritis are associated with more pain
Key Take-Away:

Hand osteoarthritis is a degenerative joint condition of hands involving severe pain and damaged functionality that negatively impacts the person's quality of life. It makes its diagnosis on correct time more critical, and this study helps out to find clusterin serum level association with hand osteoarthritis.

Introduction
This study was aimed to investigate
(a) the clusterin (CLU) serum levels among patients with hand osteoarthritis (OA) and healthy controls; (b) to compare CLU levels between patients with the non-erosive and erosive disease;
(c) to explore the link of CLU levels with laboratory and clinical parameters.

Methods
The study involved 53 controls and 135 individuals with OA (54 non-erosive and 81 erosive diseases). All patients went through hand joint ultrasound and clinical examination. The assessment of pain was done by visual analogue scale (VAS), Australian/Canadian (AUSCAN) hand osteoarthritis index and algofunctional index. ELISA (enzyme-linked immunosorbent assay) was used to measure CLU Serum levels.

Results
Serum levels of CLU were considerably higher in control subjects as compared to patients with hand OA (p < 0.0001). Besides, patients with non-erosive hand OA had substantially higher CLU levels than those with erosive disease (p = 0.044). As per VAS and AUSCAN, a negative association was seen between pain and CLU levels among erosive hand OA patients (r = − 0.275; p = 0.013 and r = − 0.220; p = 0.049, respectively).

Conclusion
The study represents that patients with hand OA exhibit lower levels of CLU as compared to healthy individuals, particularly in those with erosive disease, and that CLU concentrations have a negative correlation with hand pain.
